A 47-year-old man with obstructive jaun- dice for 20 years consulted our hospital. Magnetic resonance cholangiopancrea- tography (MRCP) and endoscopic retro- grade cholangiopancreatography (ERCP) demonstrated saccular dilatations of the intrahepatic and extrahepatic bile ducts and a type IV-A congenital cho- ledochal cyst was diagnosed [1]. He un- derwent cystexcision of the dilated extra- hepatic bile duct with a Roux-en-Y hepati- cojejunostomy. Because a frozen section of the biopsy from the hilar bile duct showed high-grade intraepithelial neo- plasia, a T-tubewasinsertedintothe com- mon hepatic duct.
